Chandigarh:For almost a decade, a couple from northern India navigated a conception journey filled with unanswered questions and continuous disappointments. They had undergone three IVF cycles and four embryo transfers – all unsuccessful – when they arrived at Birla Fertility & IVF's Chandigarh centre. Despite the setbacks, the couple was determined to achieve parenthood and chose to give one last shot at Birla Fertility & IVF.
The 28-year-old woman had been married for more than eight years. Her reproductive parameters – an AMH of 2 ng/mL and normal ovarian reserve on ultrasound – indicated no serious issue. The challenge was with her husband who had severe oligoasthenoteratozoospermia (OATS) – a sperm count of only 4 million/ml, 1% motility, and 1% normal morphology.
Although they had attempted at several other clinics, including one which still had four of their frozen day-3 embryos, the couple came looking for a different line of treatment. At Birla Fertility & IVF, their situation was thoroughly analysed by Dr. Rakhi Goyal, who suggested a strategic and sequenced personalised treatment plan.
She employed sophisticated microfluidic sperm selection in ICSI to minimise the impact of DNA fragmentation in the husband’s sperm. The outcome was six good-quality blastocysts. To avoid any possibility of chromosomal defects, the team chose preimplantation genetic testing for aneuploidy (PGT-A), which confirmed two euploid embryos for transfer.
A frozen embryo transfer (FET) cycle was scheduled meticulously. Hormonal replacement therapy and downregulation were performed to prepare the endometrial lining so that the uterus was synchronised for implantation. With the ideal thickness and progesterone levels, both the embryos were transferred.
Twelve days after the transfer, a positive beta-hCG test confirmed conception. The initial scan revealed two gestational sacs, both healthy and the patient progressed comfortably with a healthy dichorionic diamniotic twin pregnancy.
"This is the result of cumulative clinical decisions, rather than one breakthrough moment," said Dr. Goyal. "What made the difference here was not one single intervention, but the sequence and timing of multiple personalised decisions. It was also the patient’s persistence that brought us to this point. Three failed cycles are not easy to come back from, but with the right tools, timing, and a personalised approach, success is always possible.”
